  • Permanent Case Manager role - 3 days per week
  • Established and respected provider of Aged Care Services
  • South Eastern Suburbs
  • 3 days per week potentially growing to a full time role
  • Home Care Packages
  • Malvern East location

Excellent oportunity to join an established provider of Aged Care Services who value the importance of the relationship between clients, families and staff

To be considered for this role you must have:

  • Relevant experience in case management of clients on home care packages
  • Qualifications in Nursing, allied health or social science
  • Able to commit to 3-5 days per week (Mon - Fri)
  • Strong knowledge of CDC and HCP legislation
  • Experience working with a care management software package(Carelink a distinct advantage)
